Bad Breath Tongue Cleaning

How to have fresh breath all day?
I brush my teeth with an electric brush for 2 minutes, use an OraBrush tongue cleaner/scraper on my tongue, floss, and use mouthwash. Yet, throughout the course of the day I find myself having to chew gum because my mouth starts to feel not-so-fresh. What is the cause of this bad breath to come within 3-4 hours and how to do I stop it from happening?
And I mean to say that I want the fresh breath without having to chew gum and mints all day.
